ISS Forms

Many procedures in the Office of International Students and Scholars (ISS) require the completion of an ISS Request Form. International students are directed to their departmental academic adviser to complete these forms. Advisers should complete the form only for students in their own department and only that portion of the form designated for the academic adviser or graduate program coordinator, depending on the procedures of individual departments.

Advisers are reminded to consider the purpose of the request when signing the form, e.g., if the student is requesting a program extension, is the projected completion date a realistic estimate? If applicable, is a departmental funding letter attached? Such attention will expedite the student's request and reduce the need for additional follow-up between the department and ISS.
